At Prime Care Pharmacy, we offer Pharmacist Managed Anticoagulation Services for patients taking warfarin (Coumadin). Our pharmacists are specially certified in the management of oral anticoagulation therapy. We use the point-of-care device “Coaguchek” that requires a small sample of blood obtained through a finger prick test. The INR result is provided within one minute. The accuracy of the point-of-care device is comparable to lab quality results.  

Patients can come into our store for testing or if unable to make it to the store a pharmacist will come to see you. The Pharmacist will adjust the dose of warfarin therapy based on the INR result and schedule your next appointment. This system allows for timely warfarin dose adjustments and ensures prompt attention to INR values that are outside of the target range. We will report any concerns to your family physician.
